Eye Accessories Assessment

1. I can confirm I will use my Eye Accessories for aesthetic and fashion eye colour enhancement only and not as an attempt to correct any sight defect of any sort
2. My eyes are healthy and do not suffer any of the following:

• Infections or allergic of any kind
• Cornea conditions such as Keratitis
• Chronic or serious conditions such as glaucoma
• Inflammation of the eye such as iritis, uveitis
• Corneal ulcers
• Corneal dystrophies

3. I confirm that I have been given comprehensive clear advice and instructions on the fitting, wearing and removal of my Eye Accessories by an optician, optical dispenser or a medical practitioner
4. I understand that I must immediately remove my eye accessories in the event of undue discomfort, soreness, irritation or excess watering
5. I am aware of the need for regular check-ups with an optician, optical dispenser or medical practitioner
6. I confirm that sharing my eye accessories can cause eye infections
7. I confirm that I should not sleep whilst wearing my eye accessories
8. I confirm that I should not wash my eyes with water whilst wearing my eye accessories
9. I confirm that I should not wear my eye accessories for longer than 8 hours at one time
10. I confirm that I am over the age of 16 years
11. I understand that there may be a variation in colour and shade depending on my original eye colour

Please ensure that you are advised on the care of your eye accessories by an optician, optical dispenser or medical practitioner prior to purchase.

I am aware and understood that eye accessories are not classified as medical devise and are governed by the EU cosmetic Directive. They are considered to be suitable for people with healthy eyes who must confirm all the above statements to be true and accurate.
